1968 FM Fearbook cover ghoul ....HELP NEEDED

Hey fellow monster ranks, does anyone know if the beast illustrated on this copy of FM's Fearbook was actually in a film, or is it simply an original art design for the magazine?  It's lovely, but I am unfamiliar with the story behind the creature, and would like to know if he's/they are a movie star. Thanks for any help.

It's a concept drawing for the 2-headed monster in "Jack the Giant Killer" (1962).

Mike Parks from Mad Lab Models came out with the FM cover version of the 2 headed monster.  Do a google search for photos of it.  I always wanted one but never had the extra spending cash for it.  Sadly Mike died a few years back.
