Does anyone know what this mask is?

Ive wondered about this for decades. We see lots of the heads/ masks/ busts in photos of the makeup and fx shop over the years but Ive do think Ive ever gotten an unobscured look at the mask behind the Creature's head. Does anyone have a clear pic or know what it was used for?
